• HEALTH REQUIREMENTS

    State Law requires a recent physical exam and up-to-date immunization form for students entering Pre-K, Kindergarten, 6th Grade and New Students to Illinois prior to entry.

    • Physical Health Examination (performed by MD, APN, PA) MUST BE ON AN ILLINOIS SCHOOL PHYSCIAL FORM – (No other forms accepted i.e. IHSA, doctor dictation, etc.)

    • Lead screening: Questionnaire and/or blood test – According to Illinois law, all children 6 months through 6 years of age must be assessed for lead
    • Diabetes risk assessment screening per IDPH rules: BMI>85%; Family History; Ethnic Minority; Signs of Insulin Resistance

     The school district includes informational materials regarding influenza and influenza vaccinations and meningococcal disease and meningococcal vaccinations developed, provided, or approved by the Department of Public Health under Section 2310-700 of the Department of Public Health Powers and Duties Law of the Civil Administrative Code of Illinois when the board provides information on immunizations, infectious diseases, medications, or other school health issues to the parents or guardians of students.

    Pre-K:
    Immunization Requirements:

    • 4 DTP (with the last one given on or after the 2nd birthday)
    • 1 Measles/Mumps/Rubella (with the first one given after 12 months)
    • 3 Polio (with the last one given on or after the 2nd birthday)
    • Hepatitis B series (3 doses, with the 3rd dose on or after 6 months of age)
    • 4 HIB doses or one dose after 12 months of age
    • 4 PCV doses or one dose after 12 months of age
    • 1 Varicella (given on or after 12 months of age) or doctor verified evidence of disease with date

    Kindergarten and First Grade:

    • 4 DTaP/DTP (with last one given on or after the 4th birthday)
    • 2 Measles/Mumps/Rubella (with the last one given on or after 1st birthday and second dose no less than 4 weeks later)
    • 4 Polio (with last dose given on or after the 4th birthday)
    • 2 Varicella (1st dose given on or after 1st birthday and second dose no less than 4 weeks later). OR doctor verified evidence of disease with date

    2nd, 3rd, 4th and 5th Grade:

    • 3 DTap/DTP (with last one given on or after the 4th birthday)
    • 2 Measles/Mumps/Rubella (with the last one given on or after 1st birthday and second dose no less than 4 weeks later)
    • 3 Polio (with last dose given on or after the 4th birthday)
    • 2 Varicella (1st dose given on or after 1st birthday and second dose no less than 4 weeks later). OR doctor verified evidence of disease with date

    6th, 7th and 8th Grade:

    • 3 DTap/DTP (with last one given on or after the 4th birthday)
    • 1 Tdap immunization
    • 2 Measles/Mumps/Rubella (with the last one given on or after 1st birthday and second dose no less than 4 weeks later)
    • 3 Polio (with last dose given on or after the 4th birthday)
    • Hepatitis B Series (3 injections at the appropriate intervals)
    • 2 Varicella (1st dose given on or after 1st birthday and second dose no less than 4 weeks later). OR doctor verified evidence of disease with date

    Eye Exam Requirements:

    Illinois State Law requires an eye examination, performed by an optometrist or ophthalmologist, for students entering Kindergarten or New to Illinois.

    Dental Requirements:

    Illinois State Law requires a dental examination for all students entering Kindergarten, 2nd, and 6th Grade.

    Current Sports Physical is required if you are participating in a sport at the Junior High.
